Apply online for a library card today - and begin using cardholder services immediately!

Getting a library card is easy - simply fill out and submit the Library Card Application below. After submitting the application, you will receive a temporary library card number that allows you to take advantage of online services (such as Research Databases and eCatalog) immediately.

To complete the application process and pick up your actual, permanent library card, stop by Woodbourne or Centerville Library at your convenience. You will need to bring the following with you on your first visit:
  • a photo ID with current address OR
  • a photo ID and separate proof of your current address (e.g., piece of mail or checkbook) if the address on your ID is not current

At this time, only Ohio residents may register for a library card online. Out of state residents and those applying for teacher cards must apply in person. If you are Homebound, please send an email with your name, address, email and telephone number to wcplpub@wcpl.lib.oh.us and ask to begin the registration process for a Homebound Patron.

Children ages 5 - 17 may apply for a library card via the above form, but to receive their permanent library card (and gain access beyond the Library's online services) upon visiting the library, a parent or guardian must be present to provide the necessary information and address verification documents to activate the account.

Understand that, by applying to become a cardholder or being the parent or guardian of a cardholder, you are agreeing that:
  • you (and any child cardholders you are responsible for) will observe the rules of the Library.
  • you will be financially responsible for all charges for late returned, damaged, lost or stolen materials borrowed on this card.
  • you will report the loss or theft of this card immediately to avoid unauthorized use.
  • you will update your name, address, and phone number whenever necessary.
  • you understand that damage to patron equipment resulting from use of library materials is not the responsibility of the Library.
  • you understand that this card enables your child aged 5 - 17 to borrow any item from the Library collection, including books, videos, and other audio-visual materials.
  • you accept responsibility for guiding your child's use of the Library and its resources.
